So, do we know if this means users have to have the ability, or the people who run the app? Seems vaguely worded.

“in-app” means exactly that. The app has to provide the function for users.

The Apple terms say “the ability to block abusive users from the service”. No app allows a user to block another user “from the service”.

And Google has a different TOS, which I was referring to.
